I think for most the appeal is that the girl is essentially helpless in an aspect of daily functioning. Unlike plain watersports however, diapers add a further device of embarrassment into the equation.

In other cases, it is more about nurturing (more in the adult baby aspect though). A guy who would like this would like the idea of having a girl dependent on him, and since diapers are one of the most elementary and submissive forms of (non-bdsm) dependancy they work.

Of course some guys like these roles of dependent and providing switched, but IMO the general helplessness of being put into diapers is what appeals to most in the fetish.
