Give it to me straight, /jp/. How much tougher is this game if I start out without recruiting Mokou and Byakuren immediately? Every time I play through it I always recruit them because the characters are so weak early on and you could use as much firepower as you can get.

I'm mostly worried about Alice, Komachi, and Kaguya/Eirin. The latter mostly, because iirc, the only other way to get the control status besides Byakuren is Satori's last word which is an extremely unreliable method.

Not that much. I only recruited Byakuren and Mokou on stage... I think it was on the Eientei split-up. Maybe one stage later.

Not as much as you think, actually.
Alice can be easily dealt with by Marisa, Patchy and Nitori; Komachi has no accuracy so Sanae alone can help take care of her; and Kaguya/Eirin stand in a straight line, their races are magician/warrior and they have no elemental weaknesses so they're just begging for Nitori to murder them even without mind control. Another option against Kaguya is getting Lunatic Voice from the werewolves in her stage so you don't have to worry about her spells.
Getting Mokou early on almost feels like cheating to me, given how she's impervious to anything in the first few stages, has no problem killing anything even with basic attacks and you get the best formation when recruiting her.

Is this game any good if I don't really follow touhou?

The sprites looks kinda cute. How is the gameplay?

I don't think it has anything to do with touhou other than having touhou related sprites and terms in it.

the most Touhou related gameplay element are definitely the bombs, in that they are used both for defense and offense.
