Patchy is an asthmatic thaumaturgist, not an anemic one.
She does get anemia, but its either from playing with heavy metals as Marissa said or because she offers herself up to Remi for snacks.

I've never really gotten dangerously thin Patchy. She's unhealthy, yeah, but she's also not human.

Patchy would have had to look that emaciated sometime in her life. She is ageless/deathless now and for a magician to do that (according to the background works) they have to learn to survive solely on magic for a period of time. No eating, drinking, sleeping or at the end, breathing. And the last is what made teenage Patchouly do it since it meant she wouldn't die no matter how bad her asthma attacks got.
But she doesn't look that now probably- although her true figure she keeps a closely guarded secret.
She is probably rounder than both Marissa and Alice though, but that's just a rough guess.
