She was one of the people who accompanied Tsukuyomi on migrating to the moon millenia years ago and helped him build a civilization on the moon. She's one of his trusted people. Probably a great sage or even a goddess since she's based on Omoikane-no-mikoto.

I'm sure she's not just a doctor there, even the sacred Hourai Elixir is legitimately her creation.

They are not overrated, but people forget that their powers, like Reimu's, are situational and not inherent.
If they were to try to interfere in Gensokyo like Remillas party trespassed on the moon, they would have the same difficulty fighting Reimu as Reimu did them.
The from the moment they landed in Gensokyo the "impurities" would start to weaken them
They literally can be beaten on the surface by throwing dirt at them.
Their absolute worst enemy would be Yuuka. If she sent her flower hoards after them, the could not face all that pollen.
