Actually, I think they're always rotting since they're not exactly alive, but Hourai regrows their flesh faster than bacteria descompose it, at least in Mokou, Kaguya has her eternity manipulation and may be able to avoid this.

Anyways first we'd have to come to a consensus as to what qualifies as a disease and an immune system. When used in the traditional sense ghosts are ruled out as they're fucking dead and have no immune system. Youkai I have no damn clue. Are there microbes that can infect Youkai? Do "spiritual" ailments count? Among humans... Marisa perhaps? She's exposed herself to all kinds of weird stuff in that forest.
