I tip in restaurants where the waiter doesn't try to be friendly or anything. If you're anything other than purely transactional with me, you are not getting a tip. That's not to say waiters shouldn't be friendly, but they should be skilled enough to identify me as a no-nonsense sort of guy sitting in a restaurant by himself who just wants three bowls of chicken korma and two family-size naans, then to be left alone.

Most pizza places that deliver in the "US of A" actually include tip in the charge of the pizza. If you're paying by card, they'll give you a receipt that has a tip area to fill out on it just to try and get some extra money out of you.
