Practice. Plus when working with mosaics, you can tell if there is something or nothing back there. There are some artists that do CG work that dont bother but thats rarer now that CGs are less censored than they were back in the 90s.

Frankly at this point I think the artists just do it because they are too lazy to draw proper genitals on their characters.

who decides whether something gets a mosaic, a tiny black bar, or an even small white bar? if it's up to the publisher then why is there no competition to have the least noticable censorship?

I agree with this. I've seen a few hentai animes that had barely any censorship, smallest pixels ever. was called ( Alignment You! You! ) I think... It was so minimal, I could see details beyond anything else I've seen in hentai animations that is. Then you go to another hentai animations and its a giant ass circle...

Why not just go for the most minimal censorship possible like that one, and nothing but small black bars? Why so many verions of over censorship?

What if people uploaded a picture with all pixels in wrong orders so the picture would be seen as just random things. But the a program would rearrange the picture to show how it was originally. More or less like this:

1) Take illegal picture
2) An algorithm goes as follows: "take pixel n x n exchange with pixel 2n x 2n. Now take pixel n x n and exchange with pixel n^2 x n^2 where n^2 is a natural number, now take pixel n x n and exchange with pixel 10n x 10n...." Do such kind of operations sometimes (which is rather easy and doesn't require almost no processing power)
3) Upload picture
4) Someone download picture
5) Put picture on another algorithm, that is simply the reverse of the other algorithm
6) Picture is back as the original

A simple Encryption/Decryption process.
