The 16 means the number of hydrogen atoms in the molecule.

No. It only gets up to three before all further isotopes are radioactive, and pretty much have a half-life of "instant". Furthermore, that weight for Nitrogen is completely impossible- it can't be that light and still be Nitrogen.

I suspect the artist simply switched around the position by accident, and meant for those to be counts, not isotopes.

Here's a diagram of the molecule. Each corner of the rings represents a carbon atom; double lines are double bonds. The N's mean there's a nitrogen atom in place of the carbon. Hydrogen molecules are not shown since they're assumed to fill in any empty orbitals.

It's bonded to nitrogen, and non-hydrocarbon parts of the molecule are different. Most organic molecules are almost completely chains of carbon and hydrogen, so it's convenient to draw them like that.

Tetrahydrozoline is a derivative of imidazoline, which is found in over-the-counter eye drops and nasal sprays. Other derivatives include naphazoline, oxymetazoline, and xylometazoline. Poisoning can result from an overdose.

Tetrahydrozoline is widely believed to cause violent diarrhea if administered orally, as in a few drops of Visine in an unsuspecting person's beverage. However, this is an urban legend, and the actual results may vary from severe nausea and vomiting to seizures or a coma.[1] Diarrhea is not a side effect.

Tetrahydrozoline is an alpha agonist and its main mechanism of action is the constriction of conjunctival blood vessels. This serves to relieve the redness of the eye caused by minor ocular irritants.
