Depends on touhou. Humans don't smell any unusual, i don't know about hygiene in medieval Japan, but i guess it's kinda fine. Youkai, on other hand, can smell really delicious. Minoriko Aki smells like baked potatoes, bunny youkai smell like nice bunny aroma, and even clean, i'm sure they do some hygiene. Worst smell is probably Nitori, smells of swamp.

It's always like that with artists that draw this well. It's not so much unfitting as much as trying to keep the connection of manga-style artwork, however, you should be able to notice the fittingness in all of those images, this one's particularly hard since there's a strong lightning contrast between her body and her head.
