She really does care about him.

I believe it was on her route, we find out that she goes to school in the next prefecture, and has been missing class in order to talk with Shiki in the morning. (I think that was how it went, correct me if I'm wrong.) If she doesn't really love him or care about him, why would she do that?

tl;dr: Yes, she cares for him genuinely, and wants to protect him. She hides her feelings, and in one case, the bottled up feelings explode.
Only in her route (in which Shiki also considers her his most important person), she finally comes forward.

If it was shiki T doing this, it's essentially like playing a safe game of abuse , the knowledge that there is a hole dug for the eventual ill feelings helps overcome guilt. Token that the feelings are not resent or any similar feelings that stubbornly leave.
