Aya's slut meme came directly from the fact that she was the only non-Reimu touhou in many years to get an entire game all of her own. Immediately when StB came out, the joke spread that Aya got her game by sucking Zun's dick.

This was a long time before Sanae came around, and I think it set a precedent, without which Sanae would probably have gotten kinder treatment. The "girl gets game = girl is slut" thing was already set in place though, so when Sanae got the playable status, it was easy to apply the same thing to her. And when she was playable in several games in a row, that meme strengthened. Today it seems like Sanae gets called a slut a lot more than Aya, even though Aya still has two solo games, something not even Reimu has(!). In any case, it never had anything to do with Sanae's supposed virginhood or life outside Gensokyo or other Kannagi shit like that. It was the playability.

(This is also why Sakuya didn't get this treatment: the slut meme simply didn't exist back during PCB.)

Like to go with the idea that, as a personification, Komachi is indeed THE grim reaper and has personally collected and ferried every living thing that has ever died. That would also explain the somewhat ambiguous relationship between her and Shiki, since Komachi is one of a kind while Shiki is one administrator among many others.

But from the way it is written, it seems more likely that there are several shinigami and that Komachi is "just" the ferrywoman in charge of Gensokyo.
