Kinda annoys me that he focuses too much on the "it's just a trick - or is it?" gimmick lately. It doesn't really do much for me.
The thing I really love about his stuff is the girls voluntarily/semi-voluntarily participating in things that might make them physically or mentally scarred/disabled/dead. I'd prefer he stuck with lower tier stuff that's real instead. Also, needs more descriptions of girls anxious/afraid/in pain(but still willing), instead of just describing the setup, but that's impossible if the doubt about whether it's real or not is to be preserved.

That depends on the series mostly, the Cruelty High series is pretty much all fake(...or is it? Picture related), while the random pictures are played straight.

Yes, the descriptions are half the fun.
I'm not keen on the ones with just gore and dead girls either. For me, the best part is the fear and anticipation in the girls, like the one with the girl in the guillotine trying to persuade the preschooler not to set the device off. Slow suffering is also fine, but just dead bodies after an execution isn't interesting to me.
