I can bet you whatever you want that in the first draft of Tsukihime Ciel was supposed to be just an important support character, probably not even a female one idea he just rehashed in the sequel with Kotomine but somewhere along the line someone told him there wasn't enough variety in the heroine cast and he had to genderbent Ciel and half-ass a route for her that Nasu devilishly used just for fleshing out Arcueid's character.

Sakura is nothing like Ciel. Sakura's Sacchin traits are her shyness, knowing the most about the main character out of the girls (well, about his Arima days, at least) and being in a situation where her survival might lead to the death of others (Sakura even sucks Shirou's blood in Realta Nua, making the parallel even clearer). The Kohaku traits are how both like to take care of others, have an important ribbon, and of course the traumatic backstory. I guess you could also draw some parallels between Sakura and Seo, since they both have relatively little screentime and are love rivals to the tsundere who's suggested to be slight interested in them.
