Gensokyo is nothing but the delusion of two girls who were both abused by their family and have sever mental issues.

Both of them have stage 9 autism. Y'know, the kind that's infectious. Sometimes they pass by other people and infect them, so they become "residents" of Gensokyo as well.

"Reimu" (Real name is Tsubaki Sonoyama) has DLLR syndrome and constantly escapes the hospital while in her dream world. She believes she's extinguishing yokai, but she's really lighting houses on fire and throwing rocks at normal people.

Marisa is a stoner who got infected with autism and paralyzed from the waist down. She gets high on mushrooms every day and sometimes runs people over with her motorized wheelchair.

Sanae was just a school girl that was trying to earn extra money by selling her panties, selling her body to older men, etc. One day Sanae was bit by Reimu, which transferred autism directly into her blood stream. She now goes around "collecting faith" from people free of charge.

Every Touhou is, in fact, an ACT. A theatrical play. All the girls are normal human beings who live everyday lives and work as actors in the local theater. And don't think there are 110 of them or something, they are 20 at best, who dress up as different characters for each "game", for each play, and sometimes pay some schoolgirls for extras. Why do you think every loading screen says: "Girls are now preparing"? Because they are busy putting on their costumes and makeup.

Gensokyo is really just something that Tenshi made up to try to alleviate her infinite boredom in heaven. SWR was when she finally got around to making a self-insert character, which is why she's the final boss, and her story mode has her kicking the shit out of nearly everyone who beat her. This is also why Yukari hates her unquestioningly- this is the easiest kind of opposition to create for a self-insert protagonist.

Tenshi is, in her mind, her own Mary-Sue.
