what really happens is this:

the player character is a computer program based on someone that is comatized in real life. Unlike others who soul hacked their way into the game, the player has no body to back to.

At the end of the game the entire database of virtual world is deleted, the player included. Before the player is deleted he sends one last email to Rin Tohsaka to find the player's body in the real world. This will body will not share the memories of the player character, but still the player character hopes that Rin will get along with the real body.

The virtual world got to have avatars. Rin's real body is an illegitimate daughter of Tokiomi, but doesn't look like Rin. Still, in-game she looks like Rin. So Nero probably looks like Arturia because lol fanservice.
