They keep talking about killing each other in every Touhou game. Seriously, look at Sakuya in PCB. Before every boss fight she always says something like "I'm gonna kill you" and after every boss fight she says something like "have fun in hell".

Touhou makes no sense, don't try to make sense of it.

Remi's just having fun with Reimu, much like 90% of the dialog in every game.

That's because every Touhou is, in fact, an ACT. A theatrical play. All the girls are normal human beings who live everyday lives and work as actors in the local theater. And don't think there are 110 of them or something, they are 20 at best, who dress up as different characters for each "game", for each play, and sometimes pay some schoolgirls for extras. Why do you think every loading screen says: "Girls are now preparing"? Because they are busy putting on their costumes and makeup

The japanese text literally talks about murdering, but considering the spellcard rules Remilia likely just dramatized it and Reimu went along, which is why she replied so nonchalantly. "Extermination" in the case of youkai likely means "stop them from causing trouble" or something similar and ambiguous.

As long as the spellcard rules are in effect, Nobody dies during conflicts (However, I'm sure there are youkai that don't follow the rules and are thusly exterminated to keep peace). The point is that eventually everyone will follow the rules and killing will not (no longer?) be a factor in Gensokyo, keeping it a paradise. Reimu's job is to make sure Gensokyo doesn't fall into chaos, pretty much. As PCB showed, excessive chaos weakens the Hakurei border.

Because of the spellcard system you can't take words like "kill", "murder", "exterminate", "destroy" etc as their denotative meanings.

It's a good thing no one cared, or else she might've been missed.
