Not a guarantee. The conditions of going to Gensokyo are that you mingle with the residents without getting killed, nothing more. They may tolerate you, but they don't have to like you.

The real life situation, on the other hand, does guarantee waifu status. With that distinct difference, I think the question becomes a lot more difficult. There are a LOT of variables in the "go to Gensokyo" choice, whereas the real life choice is just one where you are handed a guaranteed best friend at the very least.

I wouldn't want to rip my waifu from her own world just for the sake of being with her.

Kaguya can be beaten and still has her 5 impossible task spellcards up her sleeve. Mokou (if you pay attention during the fight) DIES every time her life meter is exhausted, and immediately revives herself. She seems to be incredibly powerful in terms of offensive capabilities, but is easily killed. Kaguya has a stronger defense.

In-game danmaku battles are non-lethal. Mokou dying is the same process as your PC or the boss "dying" and throwing up some items.
Reimu joking about "going all out since Mokou can't die" doesn't count, the characters talk shit all the time, remember Yuyuko and her culinary shenanigans.

My own personal views on Reimu, plus I'd feel the same way if I had an immortal punching bag.
Besides that, Mokou complains a lot after you beat her, and a lot about the pain of it, so that leads me to believe that even if Danmaku isn't lethal, it isn't painless. Or she's a wimp.

If I were to speculate about what it feels like to get hit by a danmaku bullet, it would be a fast flying medicine ball.
Not enough to cause any permanent harm, but painful as fuck and able to induce concussion, bruises, or break ribs at worst.

If it was Yukari then I think it would work well either way, but most other character's are too deeply rooted in Gensokyo to be themselves outside it. Reimu just wouldn't be the same person she is now if she were in urban Ohio.

That said, it's still a hard choice. I'd love to meet all these people/youkai, but I also love air conditioning, central heating, and indoor plumbing. I'd definately go for it, but I'm not naive enough to think I wouldn't have regrets.

Sanae on Kanako's description says both the Kappa and the Tengu in SA extra stage as having primitive tech compared to the outside world. Ran also says they only dabble in science for fun and not for development, and especially not to further the good of Gensyoko in a page of SSiB. Apparently Nitori is a special case in the sense that she wants to advance further then her fellow kappa want to go.

Well it usually starts has having fun, but when something really stellar comes up they normally abandon it and moves on if it brings up no results. The best example was the first Gensyoko radio broadcast which was abandoned when it didn't work in a page of Bohemian archive.
