Select Unlimited from the main menu (go to new game after you've cleared Sumika and Meiya routes in Extra) and if it's not installed it'll ask you to insert the DVD with Unlimited on it.

Just did that again. Obviously, my install is fucked up. No extras show up even though I have finished the game, and Unlimied isn't translated even after installing the patch.
