No one cared much until EoSD probably, because the PC is a larger and more modern platform. It probably had its doujin fans before though.

It's been popular, but only became really so in the west when the games were finally translated.

As for its modern state, blame IOSYS for popularizing the characters and PoFV for making nearly every meme about them.

I don't know what that is, but here is a pic of Reimu from 2001. It's the earliest pic I know of and was taken from Zun's site.

It was definitely PCB. Seeing as most fanart, remixes and similar fan work are based off PCB, it's fairly obvious that PCB turned Touhou into what it is today.
