Not very often, but then again I'm mainly listening to either business materials for work (and struggling to understand half of it) or situation voice clips. So generally it's easy to grasp which word is meant based on the wildly different situations I encounter them in. That said, I do still get tripped up from time to time if I'm not paying proper attention.
