Dunno if the GiGO arcades are still open, or if they all closed? Could be worth a visit.

In Akiba specifically?
You could visit every otaku store on the strip in a single day, but it's going to be a lot of the same.
Don't miss Mandrake and Radio Kaikan. The three TRADER stores also have huge stock of 2nd hand good.
The big Animate and GAMERS are iconic but mostly stuff you can get anywhere, they're often doing events though.
If you want retro gaming stuff, shop around because prices vary massively. Super Potato is a cool store but expensive.
There are some anime/gaming themed bars if you're into that thing. Obviously loads of maid cafes and cosplay lounges too.
Just walk around the strip and go wherever looks interesting. Make sure you're familiar with Japanese business signage or you'll miss more than you see.

Also some stuff near Akiba:
Okachimachi is right next to the strip, has an interesting old shopping district and lots of small food places.
Ochanomizu is also one stop away and has it's own strip of used music and instrument stores, amazing place for music buyfags.
Ueno Park is an obvious one if you're not going otherwise, like 10 minutes away from Akiba strip.
Kinda lame but you can walk by the river to the Sumida river walking path. Quite serene by Tokyo standards, nice place to eat or drink a few konbini beers.

It's not the 2000s anymore, there's nothing you can buy there that you can't buy in literally any online shop. All the shops are dying and getting replaced by overpriced cafes targeted at Chink tourists for a reason.

Roam around in akihabara for half a day/a day just to take pictures, plan ahead for whatever the fuck collaboration atre have, be sure to visit ichiran ramen or tenkaippin, be sure to visit mandarake because they sell very cheap doujins there if you can bring home doujins, and visit kanda shrine for love live reference. If you want to waste money, then try the pachinko parlor in there, won't recommend a specific pachinko parlor place because there's a lot.
Then fuck off Akihabara because there's nothing to do there anymore. Visit Hanno for mountain climbing or yama no susume pilgrimages, and go to wareiwa bridge to see people camping or having fun in the river, though winter would be a bad time to visit that bridge, be sure to talk to the locals there because they're very kind and talkative, and then head off to chichibu bridge for anohana pilgrimage. Okay, I'm out of ideas. Limiting yourself only to akihabara because of "I love anime!" is a shitty reason to visit Japan and a big waste of time and money. I'm saying this from experience.
