I have a feeling race-mixing between youkai and humans is forbidden on the level of humans turning into youkai.
However the child isn't killed.
Case in point, Rinnosuke. He's half-youkai half-human.

I reckon what happens when you knock up a youkai is that you get to live the 9 months and up until the child is born.
Then during the night, Yukari or someone on her bidding pays you a visit.
She tells you, and your lover the following: "Your child will be fine, another family will raise it as there own and they will live a fulfilling life. Everyone is welcome in Gensokyo. However the rules in Gensokyo are "humanity must remain afraid of youkai", love is the complete opposite of that. I'm sorry, but you both have to die. Say your goodbyes to your child."

Not advocating for Grimsokyo but if nothing else, especially given the levels of power on display, Gensokyo in general could be described as an "exaggerated" version of myths, shaped more than a little by youkai cope, seeing as they kicked the bucket pretty easily in myth by comparison. That said for human villagers it's not Warhammer levels overtly but it's still pretty awful for humans.

If you romanced a youkai that's up there in terms of power and influence though, it's pretty unlikely that you'd face negative consequences for it, beyond the risk of your wife trying to rip out Mokou's liver so she doesn't have to watch you die.

What if I told you it is implied Heida no Akyuu is intentionally spreading about just how dangerous and grim dark the place is to encourage people to not step out of line and to fear youkai. If you read between the lines, your typical youkai is a total jobber and might do some tricks but isn't really a threat to normal people. The danmaku system only makes them seem stronger than they are because it is meant to act as an equalizer. There are definitely a few dangerous youkai out there, but most are actually shown to be friendly or capricious towards humans. The bigger risk is something like Kogasa scaring you while you are trying not to fall off a ledge than a roaming murder youkai.

Villagers are deliberately lied to by Akyuu and Yukari/Okina, and the village itself is riddled with youkai and spies. Touhou is fun to us because we're effectively omniscient gods to them capable of seeing everything, but if you were a regular person in Gensokyo you would barely be able to travel anywhere. Places like Former Hell, Youkai Mountain, etc. are absolutely off limits. If you ever posed a threat to the status quo as an interloper you would likely be put down.
If you're a gapped in person its extra bad. Gensokyo isn't just the youkai in the video games. You will face literal monsters in the wilderness. There's a decent chance your soul would be trapped in Gensokyo too compared to dying in the outside world.

Becoming a youkai is out of the question. The fortuneteller was foolish. Becoming reliant on human faith/fear to survive, having a fragile soul that's susceptible to being overwritten by vengeful spirits, and quite literally losing your humanity (and therefore self) is shit compared to being a normal human. Turning into a magician is still extremely shit but is at least better than what the fortuneteller did. Your best bets would be to try and cultivate psychic power, conduct the powers of a god/gods (Like Kanako), or try your hand at the hermit arts.

That's embarassing, and might ruin my plans of getting a Youkai Wive, but not life-ending.
Hell, she could do that to any man in existence, so coming from her it's just an invasion of privacy more than anything.
