I'm assuming you're referring to Dies Irae (it's not the only one with this problem). First, curse Light. If you absolutely do not care about H at all, you can just read the all-ages version. However, there is technically one detail about a character that's completely censored out for some bizarre reason. How much this matters is debatable, but the fact is a spoiler so it's hard to really discuss. I think the all-ages verison also removes anyone explicitly saying "Nazi" if my memory is correct, but there's still "Sieg Heils" (actually more technicallY) so I don't know what the fuck logic is. Bonus, some of the voice lines from the actual H scenes are reused in the all-ages with the girl obviously still sounding flustered as if she has a dick in her vagina. Go figure.

Anyways if you decided you want the H content or any of the other stuff I mention, then the answer is that you have to play both because the all-ages has several side stories in addition to another complete ending which are all essential. The jist is this: play route in the 18+ version and finish it. Open up the all-ages, and ctrl through that same route. Then play the side story you just unlocked. Switch back to the 18+ version and do the next route. Repeat. An important note is that Rea (last route) has two endings in the all-ages version. You can get the one ending in the 18+ first and finish that. However, the version of that ending in the all-ages game has one extra scene in it so considering reading that again as well. Anyways, the final big and grand ending is only in the all-ages and obviously you play that last. Not hard to figure out once you finally get to that point. Route order is Kasumi -> Kei -> Marie -> Rea.
