I took this screenshot a few months back. It's the official downloads page for 私家版, the Japanese version of eraMegaten.
The screenshot is pretty typical of what I've seen there. Major updates usually get a little over 1,000 downloads. Minor updates get anywhere from 50 to like 200.
Even accounting for people who play but seldom update, there still might be less than ten thousand people who play the Japanese version of eraMegaten. Not even all of those people are Japanese; I'm certainly not.
Although of course eraMegaten is just one game.

The games definitely have at least some cultural impact because you occasionally see references to them in Japanese media. I first found out about era games from a really hot FGO doujin by Clochette called 深海電脳楽土E.RA.BB.

Conversation choices in TW aren't anything to write home about either. The difference this could add is instead of a character's sprite getting swept away by walls of text, it can update while they speak those same walls of text. Nothing has actually changed, there's just background art and more character sprites to look at.
The real problem with creating something like this is supplying the art—preferably without directly dumping the slop produced by an image generator. Era benefits from this since all you need is a writefag to make content, but with a VN style suddenly there is a need for better/higher resolution spritework and backgrounds. I don't see anybody here making commissions for whole new sprite sheets, much less ones in alphes' style.
