Roa doesn't appear to be deteriorating like in the OG and he says that SHIKI is his best yet. That's saying a lot when Ciel was his previous incarnation. When you think of it he's got an ONI body and SHIKI's gifts as a base. Maybe they don't conflict so much in Remake. The Eyes he possesses are super useful against Arc and basically any living this as well. And he's also seemingly not full power yet so who knows how strong he could have been? His survivability alone is probably a good deal higher considering he revived from an ankle after getting MP'd and while as Ciel he seemingly had a pretty whole body when he died.

That's no longer possible with Arach's addition. I do understand your worry. One of the things I loved about Tsukihime is that once you entered into the Tohno's side of the story anything (almost) about the vampires becomes completely obsolete. It became an entire different story.
But this time Arach is around and something related to the vampires happened in the old hospital. Unfortunately that complete detachment between Near and Far side of the moon has been lost with the remake.
Also, Mio and Goto were added, they are a branch family of the Tohnos, why should they appear while Kouma no? Kouma is literally the reason why Shiki is there, his worst fear. His ultimate nemesis after SHIKI.
