Do fairies exist? Do monsters exist? Does Gensokyo exist? Do gods exist?
Most would answer no to most of these, with gods being the only ones with some faith left--sometimes a lot of faith left.
These things aren't real, but they need to be assumed real to be real. They only exist off of that. Even within Gensokyo itself, "reason" and "common sense" are threats that can wipe out near entire species of youkai (such as yamabiko).
The barrier was put up to keep Gensokyo as a nature preserve. Without it, and subjected to common sense, magic and the supernatural will perish because no one believes that shit. Of course we don't, because that's just common sense.

Yukari started the Youkai Expansion Project long ago to basically beckon supernatural beings naturally to Gensokyo (this was before the barrier) and to repel "normal" things. That's the actual foundation of Gensokyo as it's known in the modern Touhou setting, and the reason supernatural beings flock to it--it actually isn't directly related to the barrier. It's its own "boundary", and has existed for over 500 years (the Great Hakurei Barrier has existed for over 100 years)

Going out of the barrier is considered a forbidden act, but this is done by several youkai.
The youkai living in the outside world are very few and have rough lives. Furthermore the only example we know of an explicit youkai from the outside world who definitely came from there is Mamizou, a tanuki. Tanuki as supernatural youkai are some of the most famous, implying that may have sustained her.
Even so, she has nightmares about the outside world.

You can read anything to find out any of this rather than speculating based off a wiki or reddit or 4chan discussions.

SDM has never actually been explained, in the sense of "where it came from". There are other worlds aside from the outside world.

If you're wondering why youkai don't just go to those other worlds, it's because in most of them extremely powerful and extremely violent creatures exist that really don't play nice. They'll murder, obliterate, conquer, enslave etc and few can actually fight them, even from Gensokyo. This is probably why Gensokyo is often described as peaceful, and it is known Gensokyo's (relatively) nonviolent nature along with its calm natural environment are what Hecatia fancies about it (being someone who has traveled many of these dangerous other worlds)

The Japanese still have superstitions about animals. For example, some kids, even, will say "don't whistle at night, it'll make snakes come out". Foxes and tanuki are still basically thought of a bit suspiciously, I guess. I assume this is why Mamizou was able to stick around, but Mamizou also lived among humans as a human and it seems to imply she was by herself as a tanuki. She's also very old and infamous. Could be said even tanuki aren't that big a deal in the outside world, perhaps.

Anything other then an event that's completely unexplainable and catches the eye of the entire world wouldn't even move the needle and even then they'd have to push back against the overwhelming belief that magic does not exist. Just a little thought about Reimu's role and the Hakurei God:
The Hakurei shrine maiden is strong enough to content with just about everyone in Gensokyo, despite the Hakurei God being a complete enigma. Its name is forgotten, few of its rituals if any are performed, it has few worshippers, and its shrine is remote and dangerous to get to. By all intents and purposes the Hakurei God should be near powerless if we are abiding by the rules of faith the Gods are bound to, yet Reimu is still granted enough power to demolish powerful youkai and Gods. If the Hakurei God is this powerful at rock bottom, how much stronger could it be—and by extension Reimu—if it had faith and a name to pray too? As an anti-youkai God, it's no coincidence that it became forgotten as it would be impossible to control and create Gensokyo as a haven for Youkai. Reimu is strong, but just weak enough to be forced into maintaining the status quo for the sake of the continued existence of Youkai. Gensokyo could not exist if the Hakurei God was any stronger, the balance would be firmly in the Human's favor and Youkai would be exterminated.

Someone more versed in Shintoism could explain this better, but going off his "Dragon God" title + printwork descriptions, it's safe to assume he's THE Watatsumi/Ryujin of ancient myth. Basically a deity of the sea and sky who was an ancestor of the imperial family. He's still worshiped today in real world shrines by rural folks who depend on fish and good rains. So despite being relatively minor compared to all the big religions, he still has enough faith to survive in the outside world. This of course means he has to take a hands-off approach to Gensokyo. Like you said, just enough to empower the Hakurei, but still obscure to let forgotten gods/youkai thrive.
Fun fact, he's also known to take human form in some myths. He could be strolling around his little nature reserve at any time and no one would know.

A lot of people keep saying the barrier is to keep youkai in, and the magic that sustains these youkai. That magic is the belief of the humans in Gensokyo, which would be disrupted if the humans could leave, or if they would receive too much outside influence that freely traveling youkai brought in.
More of a physical than a spiritual example, but there's a chapter in Wild and Horned Hermit about completely eradicating the western dandelions from the outside world that would disrupt the delicate balance.
