How does this work though? I thought Clocktower 2015 also happened in Fate/Stay Night worlds. I was under the impression that CT 2015 is the difference between FGO happening and not happening. I don't think this necessarily means that Tsukihime and Mahoyo are in the exact same world either.

You have to differentiate between similar events and the works. Mahoyo takes place in Tsuki's world but you still have some things happening(though obviously somewhat differently) like Touko's train involvement. Rather than CT2015 being the difference, it's Lev's death itself.

I don't think Arc exists in FGO, not in any way that resembles Tsukihime Arc as much as A3 anyway. Bear in mind the entire reason Blackmore killed himself in Fate worlds was because there was no chance his master could be revived. If you had A3 Arc in Fate then that would be the optimal condition for Crimson Moon to resurrect.
