Mahoyo is a great story that explores the lives of our three protagonists. Aoko, Alice and Soujuurou.

It is a calm story about leaning to live with others and the way to follow with your life. It is a slice-of-life at heart, with some of the best content being on the “.5” chapters, which you need to go out of your way to access, since they are, technically, filler.
It possesses some great character moments and interactions, with amazing fights that serve as mid and final climaxes, in which our main character’s personalities are made to shine, as they are displayed in narration and the way they act.

It is hard to compare it to the other works, after all, this might as well be a completely different genre of story. It isn’t a tense “kill or be killed” with mysteries and relationships being explored at all moments, rather, it is about experiencing moments with the cast you love. There is some mystery and conflict, but it happens mostly off-screen, only rearing its head nearing the end.

I suppose it is a matter of personal preference whether or not you prefer it over the others.

Re-ACT is kinda a remastered version of the original so you can skip that, Act Cadenza is an arcade port of Melty but it also features new characters so it's basically a sequel, and Actress Again has received a PC only balancing and story update called Current Code so play that either through Steam or better download the Community Edition. Vlov is indeed really great.

I feel like its almost blasphemy to say it on a site named after the guy, but Nrvnqsr never had much depth. In fact, his personality aside from being evil seems to just be 'cliché anime villain who shouts BAKANA when he inevitably gets defeated'. He didn't exactly charm. Aside from his weird-ass powers, he doesn't have much going for him. Rather than the character himself, what's memorable about him are the situations he created (the hotel scene and the eventual confrontation).

In comparison, Vlov does just about everything Nrvnqsr doesn't:
1. He's more of an actual vampire, so it makes more sense why Shiki would confuse him for the actual murderer.
2. He has character, values and convictions that aren't just "I'm evil".
3. He's approached differently in different routes.
4. He actually respect's Shiki's abilities rather than just unceremoniously getting destroyed because he gets too cocky. (in fact, this feature is exceedingly rare among TM villains in general)
5. He has scenes told from his perspective, which are both terrifying while also providing insight into our villain.

And for the things that Nrvnqsr did had going for him, Vlov just amplifies.
The hotel scene is still there, and rather than shock you with weirdness, it's more ominous and horror-filled.
The final confrontation has far more going on than the original Nrvnqsr one, leading up to a much more climatic conclusion.

That is my personal theory because she seems to know him judging from her introduction but Shiki's reactions points to them being strangers as he's taken aback by her appearance and behaviour. What's even more suspicious is Akiha introducing the two of them but specifying that Dr Arach is Makihisa's old acquaintance. Arach also gains a surprising amount of info on Shiki's biology such as his hollow bones or his special muscle structure just from a simple check up. Maybe she was brought in by Makihisa after he killed the Nanaya clan to help learn their secrets and later assisted in brainwashing Shiki?
