I've finished analysing panties, and these are the exact mechanics.
Every day, each character will roll their daily panty at random, upon which it runs a check to see if that panty is valid. If the check fails, it rerolls the panty until the check succeeds.
This check can have absolute requirements including traits and skills, or roll a die and have a chance at succeeding.
In general, characters will usually wear ordinary underwear, and sexy underwear more rarely, though [Love] increases the chance for sexy underwear.
There's also a special check that reverses this behaviour, with sexy underwear being more likely and the chance of plain underwear being reduced.
This check is triggered under the following conditions:
-In the case of [Ignorant], the character will not adjust their behaviour. The check automatically fails.
-If the character plans to visit you that day and has given consent for sex or has [Love], the check succeeds.
-If the character has [Love], and it is one of four special days (White Day, Valentine's Day, Christmas Eve, or Christmas Day), the check succeeds.

To summarize the panty generation mechanism, the main unlock conditions for new panties are Love, Sadism 5, and Exposure 4.
Ignorant is ideal, and Childlike characters have the same options as them minus one. Small characters (Petite, Child-Sized, Inchling) come right after, as they also get some of those options. For all of these, you'll need to get them to [Love] to unlock most of their more adult options, however.
Characters with [Little Devil] will wear some more daring types of panties from the start, but they unlock nothing new on [Love].
[Sex Friend] unlocks some options, but disables others. [Love] has all unlocks that [Sex Friend] offers except for one ([Ignorant] characters can also get that one).

I've also looked into the labels for uncollected panties, and they seem to be a bit spottily implemented.
Main possibilities:
"Req Sadism 4": Sadism not high enough. If Sadism check is met, will say "Req Love" instead.
"Req Love": All requirements are already met and the panties are included in the daily roll. Getting them to [Love] is not an actual requirement, though it makes them a bit more likely to actually wear them.
There's also three other options that exist, but will never actually be seen as the panties are not listed in the overview before their conditions are met.
"Req: Sexfriend": Needs to be [Sex Friend]
"Req: Exposure 4/5": Needs to increase Exposure

So in order to maximize your panty hauls, focus first and foremost on teaching [Ignorant] characters the meaning of love. Do the same for womanlets.
Once those run out, bring non-[Little Devil] characters to [Love] instead.
For characters who have [Little Devil] or are at [Love], you can unlock more panties by raising Exposure and Sadism.
Bring Exposure to 4 to unlock three more pairs for everyone. {Ignorant]/[Childlike]/Small characters unlock one additional one on top of those.
Bring Sadism to 5 to unlock one more.
For a completely optimal scenario when dealing with non-[Ignorant] characters you can get them to [Sex Friend] with 5 Exposure first, before getting them to [Love], for one additional panty, but that's really not worth the effort.

All of them will give Medicine the required genitals, but Alice or Yukari are probably the most sensible choices, depending on how how much effort you want to put into it.

Switching versions didn't either. Under normal circumstances, a girl will switch out her pyjama panties for her daytime panties when they wake up.
But if you pilfer their panties while they're still sleeping, they decide to just go without for the day, instead of putting on a new pair.
However, if you first steal their panties while they're awake, then put them back to sleep, they'll put on a new pair, thus allowing you to get two pairs in one day.

I'm not sure if Satori has NG+ dialogue, though. As from what I can see, only Reimu, Suwako, Nue, and Akyuu are coded to check for NG+. Ellen has her own special trigger if you carry her lover item on your "first" meeting, but that's just three lines or so.

Since I haven't explained what this actually does yet, this is really just the Lunar Veil from SSiB, which was apparently very poorly translated.
What it does is cut down the travel time to and from the moon significantly, by capping it at 120TSP/40 minutes.
Amusingly, this also means that for some trips, such as going from the Myouren Temple to the Mountain Summit, it's actually faster to use the moon as an in-between rather than making the trip directly.

I think it's just Nue and Ellen who get special items outside of those two, though Ellen's does not seem to do anything substantial.

Other than that, there don't seem to be many character events that mess with your traits, either.
Obviously there's the [Hourai] event, but other than that, it's just Ellen giving you [Mixing Knowledge], though that seems fairly superfluous, and a special event for Luna where if you are a child, you (and her) can grow older.
Special xp boosts may be a little more common, like Yuugi's training, but I'm not going to look for all of them.
