It is implied in both ways. Metaphorically way was she was attacked and drained of vitality, but it is hinted that there was sexual things done to her while doing so. Yeah, I'm pretty sure the book gave Shinji command spells. He used one when Rider got her stomach slashed in HF. Also, Sakura used one of hers telling Rider to protect Shirou when she saved him from Assassin. And then she used another one telling her to protect him till the end. So I don't think her command spells are linked to the books she gives to Shinji.

Well not really, Sakura used her command to transfer command to the book.

That is explained in HF when the 2nd book is destroyed since if Sakura used the last command spell to create another book it would be pointless since it would free Rider from the command spell.

No, its not you dumbfuck ... the 3 command spells are special as forcing a Servant to obey (like Rin wastes one to make Archer shut up at the prologue) or doing something they could not normally do (like when Shiro used it so Saber could save him from a fall in UBW -I think that was in UBW-).

Its not required to issue normal commands or it would be a VERY short war.

You CLEARLY are, go back and read Archer explanation of the Command Spell, the 3 charges are only expended when forcing the Servant act (like Shiro ordering Saber to set a orphanage on fire) or to create a near miracle (like making Saber to appear next to Shiro despite she being at home miles away).

What Sakura asked from Rider did not required a command spell as it did not required Rider to act against her will or to do something that Rider could not do.

Also it seems you MISSED that Rider is still Sakura servant in HF True End so Sakura only used two command spells.
