As far as makers that sell to english-speaking folk, there's a few. http://www.animegaomaskshop.com has been improving in quality a lot since it started, or http://wyu.ulmb.com/ sells them as well. I know people who've bought from Dolphin Studios but you gotta be able to contact them in japanese. Unfortunately you have to be living in japan to buy from Sigma, who makes the best, imo.

It's plenty possible. It's a process of mold making -> negative cast -> fiberglass resin. there's tutorials all over in english and japanese if you look. Though one should probably be able to sculpt. I've seen them made with paper clay instead of FRP before, too.

I've only heard of the resin fumes being dangerous in the actual making process (you should be in a well ventilated area) so it seems to be okay!
As for paper-mache, you could technically do it that way, but I'm not sure how smooth you'd be able to get it. Can it be sanded? Also the strength may be an issue.

See the dark area at the top of the eyes? that's black mesh. Depending on the facial proportions, some have vision from the "eyelashes" area or even tiny slits right above the eyes where people tend to draw that fold.
