For themes, people that play on pad generally use Simply-Love or (a bit newer, but similar) Waterfall.
Note that this changes some of the timings to be more similar to ITG.
Backgrounds, you shouldn't really need, since most songs define their own backgrounds, with very few exceptions, and can't help you with characters since I've never used them, but you don't need one.

For packs of songs, most things that get released these days end up on the ITG spreadsheet:

(Note that you change tabs to change release year). There is a lot of content, so try to download things suited to your taste and skill. If you like /jp/ stuff (Touhou, Vocaloid, Kancolle, IM@S), I recommend Gpop's packs: anything newer than "Illusionary Dance Fantasy" is good.
These packs also have contributions from many other good charters, so it'll get you started in being able to find your own packs to download. It's not difficult to find good packs, but if you just download a random one from stepmaniaonline, you're probably gonna be disappointed. To start out, look for things newer than ~8 years by recognized and prolific charters.

So dumb, good job literally ignoring cbrush and wife3 designed to punish mashing/manip (the only game that does btw) or that etterna has 4-9k/b. If you actually look at IIDX charting you will find that it's complete shit, if you want good patterns play 6k or pms or sometimes pop'n charts.
If I wanted I could literally recreate an IIDX games, mechanics exactly, down to the graphics and play with an IIDX arcade controller. But there is no reason (aside from buttons and scratch), as etterna's scoring is fundamentally superior for 99% of aspects of play.
Again etterna is a game simulator, If you really want you can make it exactly like any IIDX.
