tons of shmups have cute girls, even before they were trying to emulate Touhou.

The real answer is tying stage beats to its very strong music and focus on elegant patterns that also had a focus on micrododging rather than the reflex demanding macro dodging. Touhou's characters weren't very fleshed out at the time Touhou became a big hit, and while they did help you can't say the girls carried it because that was nothing new even to shmups by 2002 standards.
