Mion wasn't portrayed as unhinged in the first part of Watanagashi at all! If anything Shion was for literally swapping places with her sister and getting away with it until finally leading Keichii to Mion working at the store to reveal the deception to crush Mion's heart, exactly what she does at the end.
Through the rest of Watanagashit there's no explicit evidence either way, it's deliberately up to interpretation. Which in my mind firmly planted it as Shion since it was established Mion was a shit liar. The part where Mion calls Shion Mion made me slightly less sure it was Shion, but it only moved me back to unclear, not Mion.
I mean you can't get away with combining "My friend has an identical twin" and "My friend suddenly started acting strange and crazy with no warning" It just doesn't work.
It would have been more of a twist if it HAD been Mion!

And that's Watanagashi. It was TECHNICALLY POSSIBLE for Shion to be the victim during the first half of Meakashi, but the SECOND she tasers Mion it's all set in stone, everything that happened afterwards didn't need to be written, the part could have ended right there.
