This guy is called クラーケ in materials, and Ciel fights the bigger spider-looking guy in two chapters called クラーケIII/IV. Judging by this mention of them in Mahoyo, they probably have something to do with Roa because it's said that he was up to something in 北海 during his conversation with Nero.
Their spider appearance does make Arach suspicious though. If we assume that Arach is here to work with Roa to attempt the 6th ritual again, then them working together to create spider clones (?) of some ancient life forms isn't too farfetched although I have no idea why they''re doing that. Maybe Roa shared his research from 北海 in exchange for Arach partaking in the ritual? The クラーケ in game are probably humans that Arach has messed with due to her 愛 or whatever.
As for Gouto, I'm assuming he's probably contributing to the ritual too considering he was Makihisa's business partner and the Tohnos are constructing gigantic underground halls complete with altars, and Arach (who has known Makihisa since uni) is supposedly an architect. So yeah, I'm sure he'd let her play with her spiders beneath his dilapidated hospital, but I'm not sure there's much evidence.

I feel like the only real deviation by this point is Arc killing Roa early, and hence would've scouted areas before day 8, and possibly the destruction is a result of her jealously from seeing Shiki help Ciel.

Why she went there and why such destruction was cased is a mystery and it's hard to see how that would relate to the far side indeed. But if there was no far side relation, it seems even stranger that they would bother to make a visible difference in the CGs. It could also be some other unknown event that just only happens in Ciel's route for some reason and would presumably happen in one of the far side routes. Yeah, that's what I am saying, it was a result of searching for Roa. The timescale would put it at day 6, as she kills Roa at the end of that day. It of course follows day 5 which is when Shiki teams up with Ciel against Vrove.
Mario notes to Arc that she's being much quicker than usual, her motivation to hunt Roa was likely down to her anger and jealously from the previous day and the damage to the hospital was porbably a result of the expediated hunt.

A few pages into the prologue it skips forward a few days, the window CG changes to a different location and Shiki goes through the process of remembering where he is again.
I can't remember where exactly, but somewhere in the game a cg flashes on screen which is an edit of the initial window CG, except red and all bordered up. This is probably the 'darker room that smelled more like medicine' that Shiki remembers after waking up that a few people visited including (probably) Arach.
The different window CG and alternate red version just seem so deliberate that 2 different hospitals seems like a possibility. In any case, Shiki needs to experience some kind of tragedy at Saiki hospital somewhere in his past. I think remake Shiki is on a multiple levels of memory manipulation.
