In my experience, a lot of "Japanese only" situations arise not from xenophobia per se but Japanese people being terrified that they'll have to try to speak English, or that their Japanese won't be understood. There are doctors that won't see foreign patients for example because they don't speak English and are worried the patient won't understand what kind of treatment they're consenting to or whatever
