I like One Can Grill the Devil on an Iron Grill as a sum total.
Niy's stuff is also worth mentioning.
For art, probably Zazenbeat's pre digital stuff.
In comedy, I can't pick at all. One of Ennui Akadako/Fuantei/Bomber Grape.
In porn, probably Sakuflan or yassy's Sakuya/Youmu one.
No idea for yuri. Maybe Suki Yuri or One Shot One Kiss.

Also I wish it was easier to find doujinshi. Some circles plain don't do digital, don't show up on the usual second hand vendors, don't put anything up except way down in their twitter feed for SOME of their releases, and it's a giant pain in the ass trying to find them.

I don't know if there's anything to "get" besides the emotions that it inspires. The plot itself is just a string of events that lead to each other and ends up going nowhere and resolving nothing. There was some disaster that resulted in a lot of people dying and we're told that the world has ended. But despite the world "ending" things seem to be fairly normal besides that collapsed bridge that no one seems to be repairing. Take another look and you'll notice that besides Joon's coworker who may or may not be a youkai there's no non-Gensokyo people depicted at all, it's just a total ghost town. There's also lines like "other maid cafes HAD real magicians?" and "no one's making any film these days" and we're shown that scene from Planet of The Apes multiple times which makes it seem like "the end of the world" was all of humanity besides Gensokyo's residents being wiped out.
Now, that's a wild claim to make when we have Aya talking about people in Shanghai and stocks, but look at the title and you have something more to work with. You can see with Marisa referring to it all as an incident, Aya talking about books letting the world exist in your head, and Eirin lying to herself about being "fit as a fiddle" that there's this collective desire to ignore truth, to imagine that things are different, because even if the world's ended they can't let their fantasies end.
Speaking of fantasies, let's talk about the sea. The sea is constantly mentioned throughout, it graces the first and last pages, and appears to be on the cover, washing onto some urban area. For someone in Gensokyo the sea is something from legends, it's something that doesn't exist and can only be imagined. Reimu and Marisa have this illogical desire to go see the sea, Aya claims that people are building a city on top of the sea, and Yukari talks about seeing it as well. You could say that it's a cornerstone of the fantasy, it's what drives characters to keep on going and acts as a way to pretend that there are still people out there.

And then there's Joon, the star of the show. She appears on more pages than anyone else, and has the cover all to herself while standing in that beloved sea. But everything about her goes directly against what I've been saying about the rest of the cast. She's the girl without dreams, the one character who's seen the sea, and yet not only does she hate it but she prefers Gensokyo, their reality, to it. Then she goes on to say that the sea should only be seen in films, that this fantasy should only remain in fantasies, which contrasts Aya speaking of using books, a fantasy, to keep the world, reality, alive in your head. Next let's talk about the appointment with Eirin. Joon, the one character totally grounded in reality is getting psychiatric help from the most unstable character. Seems like the perfect setup for a gag really, but she serves as a sort of twisted version of Joon. The fantasy is dead for her, but she openly lies to others and herself to try to pretend that it's alive and uses heroin to cope with it all while also trying to keep it alive for others with her treatment, even Joon who she recognizes as having the same "sickness" as her. It's easily comparable to Joon, whose fantasy is also dead but she doesn't lie about it, relies on a weaker drug to take off the edge, but still tries to keep the fantasy alive for others by telling Reimu and Marisa to not go to the sea.
Finally, there's Yukari. I don't know how to interpret her scene, she speaks a few lines across 2 pages and then that's the doujin. Maybe she killed them, maybe she took them to the sea to ruin their innocence or for some other reason, maybe she caused the "incident", who fucking knows you lost me there I can't tie a nice bow on this analysis because of this useless hag.
There's some other things I didn't touch on or go into depth on like some odd lines here and there, a further analysis of Aya's scene, how Reimu and Marisa act as a foil to Joon, or Joon's interest with Planet of the Apes because I couldn't think of a good way to segue into them and I'm incredibly tired but it's alright because this is a flawed interpretation that relies heavily on circumstantial evidence and is easily dismantled by the idea that the author simply didn't draw any background people because the entire thing was incredibly rushed as evidenced by the scribbles that make up a good amount of backgrounds when the author actually bothered to even have backgrounds, and by his own admission that it was finished right before the deadline. Also the entire inclusion of Planet of the Apes can just be explained by the scene pictured in the doujin being the cover of the music album inspired this entire work and could have no deeper meaning than that. That's a much easier explanation than everything I typed up over the past hour.
In the end I feel like the main point of this doujin isn't some weird auteur plot which may or may not actually be there, but rather just the "emoi" it inspires in you.

For some, it's just what stands out from the rest. That's really the simplest way I can put it. It spans all categories, and is open to debate. For others, it's specific points, like how a not so known and loved character gets a bit of spotlight and has their own story touched upon by the author as they come up with little intricacies to make them likable, or give people a reason to actually pay attention to them. Overall, it really depends on what people want out of a doujin, or out of doujin material.
