It's said that Matarajin first revealed himself to Ennin, who was an important figure in the Tendai school, while he was returning to Japan from China, in the middle of the ocean.
He heard a voice which introduced itself as Matarajin, a God of hindrances/impairment, and demanded to be worshiped or else he would prevent any hopes of passing on to the next world/achieving enlightenment.

Supposedly, half of them became tengu in the mountains and half gathered under Buddhism, worshiping Matarajin.
In light of all this, Matarajin is a god for minorities/impaired people/discriminated against people. ZUN strongly emphasises this aspect of Matarajin, and this is what Okina's wheelchair represents. The whole 'back door'/'hidden' thing is also drawing on the same kind of themes.
