What's a good key config for a keyboard in PoFV, which doesn't put charge or focus on the same key as fire? Having to repeatedly pound the Z key isn't healthy or relaxing.

I never had a problem with the default. Maybe try switching to a pad.

Unless the opponent is Medicine or you play on Easy Mode, it's never necessary to go out of focus anyway, so why bother?

Sometimes it's nice to move quickly while firing.

If anyone wants to say 'no, it's not possible to have the fire key just for firing on a keyboard' and put me out of my misery, that's fine too.

To the best of my knowledge, you can charge with the fire key or charge with the slow key. You can't give all 3 options their own keys.
