Remember, her hax of time is more than just STOPPING it. She manipulates space as well, and can choose to merely slow down or even speed up time.

She can even "reverse" time, in the sense of reversing motion. She can't undo what has already happened.

Think about it this way.

Aging is a function of metabolism (all the chemical reactions needed to maintain life)

The muscle contractions, breathing, everything Sakuya needs to do (as all humans do) to survive and move need to be carried out.

If she's moving, throwing knives, and breathing (or generally being not dead) within stopped time, her metabolism must keep on truckin'
Because of this, she is also aging (albeit imperceptibly, you see what I'm getting at) relative to the number of movements she makes, breaths she takes, etc etc.

She could merely be moving at such an incredible rate as well as heightening her awareness to the nanosecond ranges. Doesn't mean she'll age at second ranges even if she's experiencing it as such.

Muscle contractions, coordinated movement like knife throwing, are electrochemical reactions, and the work done throwing knives or moving will be reflected in it's own small way in her aging the way the actions any of us do is.

Muscle contractions use oxygen; Oxygen is supplied by breathing.

The only reasonable exception I can think of to this is spell card use/flying. I can't prove beyond a reasonable doubt that these are powered by human metabolism or energy in any way other than magic, so I can't say these actions will contribute.

Pretty much irrelevant. Other than chemical changes to the brain supplied by puberty and brain development as a child, all mental development/changes are supplied by experience. The Hourai Elixir locks you into your current age, so puberty/early brain development doesn't really factor in (I think Kaguya was most likely past this age when she took the elixir, but I can't verify this).

What I'm saying is that you can be a really "ancient" immortal as in the number of years you're immortal, but still be of a young mental age, or vice versa.

If Kaguya knew how to doll-craft, she could, though. Kaguya's manipulation of the ephemeral allows for instantaneous movement like Sakuya's.

Either way, Sakuya is the one forcing herself around. Possibly with her manipulation of space.

Sakyua can cause a flower that takes 60 years to grow flower instantly. She can manipulate how fast things age, so there's no reason to think she doesn't do that for herself.
