I wasn't a fan of what I read of Ruitomo. That said, why in detail I can't tell you anymore. Only that routes were disappointing and anticlimactic or something along those lines.
I also didn't like the trap protagonist. And that's coming from someone who enjoyed tsuriotsu. I think it was how 95% of the game wouldn't change whatsoever if the protagonist would've been a girl, or something similar to that.

Oretsuba is first and foremost a comedy VN. The real meat of its content is made up of comedy skits undergirded by an interesting thematic skeleton that definitely brings a unique flavor to the table. Comedy carved out of easy and amusing rhythms of daily life of quirky characters. Barely any ups and downs to color the narrative. Rarely any twists and turns to catch you off guard. Which isn't a bad thing at all. Oretsuba is very slow and meandering. Which isn't a bad thing at all. An illusion of slow build up that delightfully meanders and disappointingly weakens, and unceremoniously tapers off into nothingness and dissatisfaction everytime you finish a route. Everything else in this VN bends to the will of the comedy to the point of frustration. The expectations amassed in tandem with the magnificent unfolding of the story only come crashing down at the sight of how pathetically the story gets resolved by the end. The flavorful prose, and the colorful cast of quirky and vibrant characters don't end up being constituting elements of something good.

I liked Natsuko's better in the original but it's a matter personal taste on that.

Those are all I remember semi-well, though I recall thinking “everyone but Natsuko got screwed up in this version” so there were probably also things changed with Kurumi, Reiko, Kaori, and Yayoi. (Might have just been fade-out-in-the-middle sex scenes annoying me on some of them.)
I know they made Kurumi call you “senpai” as if she were a year younger than you, which was strange since the part of the cast that’s still in high school were all, including the protagonist, supposed to be in their last year, about to graduate and thinking about their future. There weren’t characters younger than the protagonist in Doukyuusei 1.

Almost every sex scene was either removed entirely, changed to be interrupted midway through, or faded to black in the middle instead of showing the whole thing. (Natsuko being the exception; she actually got a full beginning-to-end scene)

Every ending in the game is dramatically different from the original except for Natsuko, but the endings are one thing I don’t have complaints about. Dou1’s original endings were super simple “and then we had lots of sex, like you see here” three-sentence-long affairs.
