Kotomine's main problem is that he has a wrong idea of what doing good is all about. You don't do it because it causes you pleasure. You do it because it's what you're supposed to do.

Kotomine has the "supposed to do" part nailed down, until the grail war he was apparently a model priest. But his problem was that he kept asking why the fuck he doesn't get any joy out of it. Somebody needed to tell him he's not supposed to. Doing good feels bad. You still have to do it.

Doing good should make you feel good about doing it. If you're not getting any benefit out of it, then there's no point. Self-denial doesn't work and is quite unhealthy. What is healthy is finding out why something is what is is in your life, and figuring out how you can change it for the better.

Nothing Kotomine ever did ever brought him any satisfaction or understanding. So, he sought to find it in the only way that did bring him some form of feeling: making other people's lives miserable. He sought to change himself, but found that it wasn't something that he could do, so he sought to understand why he was like he was.

That means he was on the quest for self truth.
