I think Kaguya deserves a game of her own. She's the most interesting character and I think all the other characters are boring cliches and fetish-fueling stereotypes that belong in a museum, not in a award-winning game such as Touhou. ZUN-sama should exercise rational thought more.

Touhou Tea Party.

You pick a character and have tea and sweets with other touhous, sharing idle gossip and stories. During this time, you have to be careful about your choice of dialogue which will affect your influence aswell as theirs with the other girls. It would be a social interaction game.

Flan.

She's hungry but no one seems to be in the mansion, so she goes outside to explore on her own. She runs into and "plays" with Touhou characters she's never met before until finding Marisa, at which point she's taken home. When they return to the SDM they discover an intruder, a new Touhou girl I guess, and she tells Flan that everyone else in the mansion's gone because I haven't figured that out yet then cue the boss music

Anyhow, game would be about being the bad guy for once. There would be several scinarios to choose from (corresponding to the variosu windows games), where you initiate an incident and try to stop Reimu or Marisa from defeating you.

Since you are the ultimate boss, you control the entrance of faries into the level, always trying to put in a combination that your AI adversary cannot dodge. You spend "minion" points doing this.

At the end, you get to take over and directly play as the stages final boss, choosing the direction of their various attacks and when attacks/spell cards are initiated. You wont have much control over the exact pattern with some spell cards, but you can choose what part of the screen you are on when its initated, thereby making this harder for the AI to doge, possibly.

Sanae and Suika in a search for missing Reimu. Maybe Marisa joins in too, just because. Also they are all wearing shrine maiden outfits, Suika looking like Reimu and Marisa with the usual black-white maid motif.

Reimu is really off in the human town looking for a date so she can begin working towards making a successor, and the extra fight is her killing you for ruining things.

Kaguya has a ditzy facade but is actually pretty smart. Constantly (ab)uses Eirin for the purpose of furthering her plans.
Reisen is the butt monkey.
Tewi is the prankster.
Mokou desperately tries to keep up a jerkass facade despite it being totally against her core personality.

Seems to jive pretty well with canon to me.
