I'd say Touhou is still gathering momentum in Western countries, but has probably reached everyone it's going to reach in Japan. Like Marimite it has a dedicated relatively small following that produces alot of crazed fan work.

We're a long way off from reaching Naruto levels of popularity and backlash here in the west and I can't see that changing any time soon unless someone makes a successful Touhou anime and localises it to a American audience. Personally I doubt that'll ever happen, it's the wrong formula for that kind of market.

I suspect Touhou will cap out here eventually as much as it has peaked in Japan. It already attracts a sizable chunk of new followers on youtube and I don't see that slowing down for a while longer, but with ZUN holding back on a Touhou anime (fan work aside) and little expansion on the distribution of his games it'll probably never reach the kind of attention bigger titles enjoy. It'll always be a oddity with a dedicated group of crazed fans.
