Kotomine isn't misunderstood, nor is he confused. Kotomine is very aware of what Kotomine is. Medea wasn't all bad. She got fateraped by Aphrodite and Hera. She did some okay stuff like healing Hercules and fortune telling some important events.
Even the child killing thing wasn't revenge it was a mercy kill on that the king of the country would have tortured them to death and hunted them to the end of the earth.

Like I said, he wasn't fooling anyone important. Rin didn't like Kotomine from the beginning. Saber didn't like him. Caster thought he was atrocious. Shirou's dislike for him was immediate.

The only people he managed to fool were his adopted father, who was too much of a good guy to see the truth in anything; Tokiomi, whose arrogance was matched only by his genetic predisposition to screw up at the wrong moment; and Zouken/True Assassin, who overlooked the obvious because they weren't really that clever, in the end.

And Kotomine was never confused about who he was in reality, he just never accepted that he could live like a villain and give into how he really felt until much later in his life, because he thought that was morally incorrect to do so, which it was. Which was why he was searching for the answer to that.
