It could. ZUN likes some fan works, so if he appreciates them enough and thinks that the ideas expressed in them are suitable for the canon, he could always add it to the canon. Touhou is still a living form of media not just from doujin/content creators but also because ZUN, the creator and author of Touhou, is still alive and creating official Touhou media.

There was the case of people thinking Remilia had a physical Gungnir based on Flandre having Lavaethin. When she was put in the fighting games, they made a physical one and ZUN, who never intended her to actually have it, approved it.

Going by Danbooru alone, this would appear to be true. The first picture of Remilia with her spear was posted in 2005, while IaMP came out in 2004. However, there are plenty of variables in this and places that could be sources of error.
