The top tier is the THIRD ROW DOWN. The two tiers above that are for characters whose power levels are not easily quantifiable. Kanako and Suwako, for instance, are goddesses. Their power level is variable based on how many followers they have. Therefore, they get their own tier because it's hard to place them in the others.

According to ZUN, the time limit is due to danmaku rules. In a no-holds-barred situation she could presumably maintain Fantasy Heaven mode indefinitely. Reimu may not be able to defeat every opponent in a real fight, but none of them could defeat her either, unless they took her by surprise.

There are other factors involved with their high power level as well, such as age (because with great age comes great power in Japanese folklore), amount of (real) battles fought, and knowledge gathered from the human world as well. And Kanako is obviously more powerful than Utsuho due to the events in SA.

So our two goddesses really do deserve their top tier ranking.

I disagree with the chart, but I have very little basis in my statements. So I stop here.
