So I guess this conflict in Hell is going too actually end up being a major plot point after all. I wonder if its going too stay contained solely too the manga or if its going too tie in with a game. Im not expecting Touhou 17 too come from this, since ZUN has never actually foreshadowed main games in the manga before any sort of official announcement. Plus, the whole thing so far seems too not be Gensokyo's problem in the slightest and so no need for anyone too get involved. A spinoff seems pretty on the table though.

Looking back, it was also kinda alluded too in Hecatia's AFiEU interview. Three Worms (or Three Corpses, as they are otherwise known) in Taoism are spitits that dwell in lower, middle and upper dantian (cinnabar fields, Taoist chakra-equivalents, broadly speaking). Basically they are evil spirits who dwell within one's body and want it to die as soon as possible. At certain dates, when the host sleeps, these spirits travel to the Heaven to snitch on their host, recounting his misdeeds to the Director of Fates, which leads to decrease in lifespan. There are various methods to stop them from doing this.

Looking into it it's... a made up word? 捨虫 (Shachuu). I only get touhou results from googling that kanji, and apparently it's made up of "getting rid of" and, eh, "bug". Not necessarily worm... Just bug, mushi. Looking into it it seems to indeed, at the very least, be unstated but definitely related to the Three Corpses idea of bugs/malevolent parasites in the body that cause eventual death, and I suppose the idea must be that magicians get rid of two of the really "bad" ones. I do wonder if this is ever explicitly stated in canon, though. It seems more heavily implied.
