I'm pretty sure they are, at least in the present. "Higher rank variety" is the same sort of language used for monsters like medusa and echidna in relation to the lamia.

That is they would have started out as a mutation of the regular orc, just like how hobgoblins are mutations of goblins, and over time they became a new functioning race as the mutations became ever more likely to birth the same mutation. World Guide 1 explains how that sort of monster evolution works.
