She doesn't, she's just bantering with them. "Exterminate" was never meant to be taken literally. She wants to beat them up so they stop causing trouble, it's her job and it's what she was born to do. In a world without spell cards she would probably have to kill hem, but we're in Gensokyo, so they can rampage and Reimu can stop them. Reimu is above all else a keeper of the balance and when youkai or celestials or lunarians get uppity she is there to beat their face in. But not kill, she never kills unless you're Orange I guess.

Reimu above all else wants peace and quiet and to just sip tea and collect donations. But she likes to fight and needs to fight to be entertained. Marisa is her truest and greatest friend that she can be normal and enjoy life with. But she likes being combative and blunt and the youkai allow her to feed those feelings.

Considering Lost Progress, it's a little funny sending the player back to the beginning of the game after making it to the third stage and having them work their way back to and past the third stage. Luckily there's no boss in the second run. A free stage to exterminate fairies and get to full power in preparation for the fourth stage!
